The following applications will be accepted: for passport renewal and issuance, re-registration of civil acts (birth, marriage, divorce); residency declarations; applications for the reinstatement of the citizenship of the Republic of Lithuania; documents for consular legalization and issuance of consular certificates; as well as other consular services.
To make an appointment, please call the Embassy of the Republic of Lithuania in Ottawa at tel. 613 567 5458 ext. 22. Indicate your name, surname, personal identification number or date of birth, the consular service you require and your preferred appointment time. Please note that consular services will be provided by advance appointment only.
Notice to individuals with appointments. Please bring photocopies of all documents for submission. Birth, marriage or change of name certificates and other documents issued in Canada must be certified by a notary public and submitted together with a translation into the Lithuanian language. During advance registration, additional information and requirements will be provided by phone at 613 567 5458 ext. 22 to persons interested in receiving the above mentioned consular services.
